- Charter
School Application*
To apply for grades K-6 print out and mail this application
to 837 E. Parker, Lakeland, FL 33801. Kindergarten applications
for the 2008-09 school year will be accepted beginning March 1, 2008 - envelopes
must be postmarked on or after March 1, 2008.
Applications for all other grades are accepted year round.

Information Sheet*
To be completed as part of the enrollment process, after the student has been accepted.
- Student
Directory Form*
This form gives permission for a student to be included in our Student Directory, which lists students/parents' contact information. To be completed as part of the enrollment process, after the student has been accepted.
- 2008-09 Enrollment Agreement
This form outlines the preschool tuition rates and guidelines for the 2008-09 school year. The form is not required to be submitted until after the student is accepted into our program.
Employment
Volunteer Application Process
- School Volunteer Application
A $23.00 fee is required for the Polk County School Board
to process a volunteer application. The School Board processing
can take up to a month therefore please contact our front
office early in order to complete the sign up process. Volunteering
can begin after approval of the application.
